FINANCIAL AGREEMENT

To provide the highest quality of care, it is important that you are informed about and agree to our current fee structure.
Please review the following information carefully.

Fees and Payment:
–  A comprehensive list of our services and associated fees is available upon request and at the reception desk.
–  Payment is expected at the time of service unless prior arrangements have been made.
–  We accept various forms of payment including cash, credit/debit cards, and electronic transfers.

By engaging with our clinic’s services, you acknowledge that:
– You have been or will be provided with the opportunity to review our current fee list.
– You agree to pay the fees for the services provided to you or your dependents.
– You understand that any changes to the fee structure will be communicated in advance and that it is your responsibility to review these changes.

If you have any questions or concerns regarding this agreement or the fee structure, please do not hesitate to discuss them with our staff.

We also note that you must meet certain Medicare criteria to access injection and infusion therapy under the PBS. Your Neurologist will assess your eligibly on the day of your consultation.
