EMAIL COMMINUCATIONS AGREEMENT

At Migraine Specialist we take your privacy seriously. For non-urgent matters you have the opportunity to communicate with us via the convenience of email. There are some risks associated with the use of email that we want to ensure you are aware of and understand before deciding whether to communicate with us via email.

RISKS:
Some of the key risks associated with use of unencrypted e-mails are as follows:

•  Your e-mail may be accessed and/or forwarded to others by another person with access to your e-mail account
•  E-mails can on occasion be sent to unintended recipients
•  E-mails can be targeted for spreading of computer viruses
•  Copies of e-mails may still be accessed even after deletion

PLEASE NOTE:
If you elect to use e-mail for corresponding with Migraine Specialist:

• We will endeavour to read and reply to your message within two working days
• It is important that you advise us of any changes to your e-mail address or changes in your decision to use e-mail for non-urgent correspondence.

PATIENT DECISION & ACKNOWLEDGMENT
• If you are concerned about the risks of using e-mail you can communicate with us via postal mail or via phone where possible.
• However, if you would like to communicate with us via e-mail for non-urgent matters, your consent will be obtained upon Patient Registration.
